GENERAL INFORMATION
Osmanoğlu Hospital, Sağlık Hizmetleri A.Ş. Your personal health data that you will
share with; Within the scope of the Personal Data Protection Law No. 6698 (KVKK) and the
Regulation on the Processing and Ensuring the Privacy of Personal Health Data No. 29863;
Within the framework of the activities carried out by the Company, this Personal Health
Data is processed based on the principles explained in the Patient Informed Consent Form.
DEFINITIONS
Explicit Consent: Consent regarding a specific issue, based on information and expressed
with free will
Anonymization: Making personal data impossible to associate with an identified or
identifiable natural person in any way, even by matching it with other data
Personal Data: Any information regarding an identified or identifiable natural person
Personal Health Data: All kinds of health information regarding an identified or identifiable
natural person
Personal Data Owner: The natural person whose personal data is processed and who is
considered as the "relevant person" in the KVK Law
Processing of Personal Data: Obtaining, recording, storing, preserving, changing,
rearranging, disclosing, transferring, taking over, making available, classifying or using
personal data by fully or partially automatic or non-automatic means provided that it is part
of any data recording system. Any action performed on data, such as blocking

philosophical belief, religion, sect or other beliefs, appearance and dress, association,
foundation or union membership, health, sexual life, criminal conviction and security
measures, as well as biometric and genetic data

HOW IS YOUR PERSONAL HEALTH DATA COLLECTED?
Your personal health data; In accordance with the KVK Law and relevant legislation;
Based on your statements or - Headquarters employees, medical companies/doctors, T.R.
Identity Sharing System (“KPS”) provided by the General Directorate of Population and
Citizenship Affairs of the Ministry of Internal Affairs, Social Security Institution, T.R. Through
individuals and organizations such as the Ministry of Health, health institutions and
organizations; It can be collected verbally, in writing or electronically.
The Company collects my special personal data, including my personal health data, from
doctors, all other healthcare personnel and authorized institutions (e.g. Private Hospitals,
Polyclinics, Private Health Insurance Companies, Family Health Centers, Public Hospitals,
Nursing Homes, Republic of Turkey Ministry of Health) above. I allow it to be collected by
the methods described above and processed within the framework described above. As
explained above, I consent to persons who are under the obligation of professional secrecy
to transfer my relevant examination, examination, analysis and treatment results to the
Company.
COLLECTION, PROCESSING AND PROCESSING PURPOSES OF PERSONAL DATA
In order to provide you with high standards of service, we obtain your personal data
verbally, in writing, visually or electronically, through the Call Center, internet, mobile
applications, physical locations and similar channels, depending on the nature of the service
provided.
In this context, the main general and special personal data, especially the personal health
data required for the execution of all medical diagnosis, examination, treatment and care
services and obtained for this purpose, are listed below;
Your identity data such as your name, surname, TR ID number, passport number or
temporary TR ID number if you are not a Turkish citizen, place and date of birth, marital
status, gender information, and a photocopy of the TR ID Card or Driving License you have
submitted,
• Your contact data such as your address, telephone number, e-mail address,
• Your financial data such as your bank account number, IBAN number,
• Your health and sexual life-related data obtained during the execution of medical
diagnosis, treatment and care services, such as your laboratory and imaging results,
test results, examination data, prescription information, which you submit for
tracking in your file,
• Your answers and comments to the satisfaction surveys you share in order to
evaluate our services,
• Your closed circuit camera system video recording taken during your visit to our
hospitals,
• Your private health insurance data and Social Security Institution data for the
purpose of financing and planning health services,
• If you use the parking lot and valet service, provide your vehicle license plate.
• Your other data that you share when you use the online services on our website
www.osmanogluhastanesi.com.tr and when you contact us via e-mail, call center or
other channels.
Your "Personal and Special Data" mentioned above will be stored physically and
electronically within Private Osmanoğlu Hospital and external service providers with great
care and compliance with the legislative provisions.
For what purposes is your personal health data processed?
Your personal health data;
• Protection of public health, preventive medicine, medical diagnosis, treatment and
care services,
• Fulfilling legal and regulatory requirements,
• Financing your health services, covering your examination, diagnosis and treatment
expenses by the Patient Services, Financial Affairs and Marketing departments,
sharing the requested information with private insurance companies within the
scope of eligibility inquiry,
• Informing you about your appointment through our Call Center and Digital Channels,
• Confirmation of your identity by Patient Services, Our Health Professionals and Call
Center departments,
• Planning and management of the internal functioning of the institution by the
Hospital Management,
• Conducting analysis for the purpose of improving healthcare services by Quality
Management, Patient Experience, Information Management Systems departments,
• Providing training to our employees by Human Resources and Quality Management
units,
• Monitoring and preventing abuse and unauthorized transactions by the Audit and
Information Systems departments,
• Carrying out risk management and quality improvement activities by Quality
Management, Patient Experience and Information Systems departments,
• Invoicing for our services by the Patient Services, Financial Affairs and Marketing
departments,
• Confirmation of your relationship with institutions contracted with our hospital by
the Patient Services, Financial Affairs and Marketing departments,
• Answering all your questions and complaints regarding our health services by the
Hospital Management, Patient Experience, Patient Rights and Call Center
departments,
• Taking all necessary technical and administrative measures within the scope of data
security of our hospital's systems and applications by the Hospital Management and
Information Management Systems departments,
• Participation in campaigns and provision of campaign information by Marketing,
Media and Communication, Call Center departments, designing and delivering special
contents, tangible and intangible benefits on web and mobile channels,
• Measuring, increasing and researching patient satisfaction by Hospital Management,
Patient Rights and Patient Experience departments,
• It is processed for the purpose of carrying out education and training activities by the
educational institutions with which the institution cooperates.
It is processed for the purpose of fulfilling obligations such as storing, reporting and
informing information stipulated by administrative authorities in accordance with the
relevant legislation. Adequate precautions determined by the KVK Board are also taken in
the processing of your personal health data. I allow the Company to process my special
personal data provided/to be provided in the future by me and third parties, including my
information written in the form above, for a period determined by the legislation or
proportional to the purpose for which they are processed.
WITH WHOM AND FOR WHAT PURPOSES ARE YOUR PERSONAL HEALTH DATA SHARED?
Your personal data, Health Services Basic Law No. 3359, Decree Law No. 663 on the

• The health institution to which the patient is referred or to which the patient himself
applies,
• Your legal representatives that you have authorized,
• Third parties from whom we receive consultancy, including the lawyers, tax
consultants and auditors we work with,
• Regulatory and supervisory institutions and official authorities,
• With your employer,
• It can be shared with our suppliers, support service providers, archive service
providers and business partners whose services we benefit from or cooperate with
(for more detailed information, you can obtain information by contacting our
hospital in writing).
I allow the Company to transfer my special personal data to relevant persons, institutions
and organizations, and the Company's subsidiaries, joint ventures, shareholders and all
their affiliates, as explained above, within the relevant legislation or for a period
proportionate to the purpose for which they are processed.
WHAT ARE YOUR RIGHTS DUE TO THE PROCESSING OF YOUR PERSONAL HEALTH DATA?
In accordance with Article 11 of the KVK Law, as a personal data owner, you can apply to the
Company regarding the following issues:
• Learning whether your personal data is being processed or not,
• Requesting information if your personal data has been processed,
• Learning the purpose of processing your personal data and whether they are used for
their intended purpose,
• Learning the third parties to whom your personal data is transferred domestically or
abroad,
• To request correction of your personal data if it has been processed incompletely or
incorrectly, and to request that the action taken in this context be notified to third
parties to whom your personal data has been transferred,
• Requesting the deletion, destruction or anonymization of your personal data in case
the reasons requiring the processing of your personal data disappear, and requesting
that the action taken in this context be notified to third parties to whom your
personal data has been transferred,
• Object to the emergence of a result that is unfavorable to the data owner by
analyzing your processed data exclusively through automatic systems,
• If you suffer damage due to unlawful processing of your personal data, you have the
right to request compensation for this damage.
In accordance with Article 13 of the KVK Law, you can submit your requests regarding these

PRIVATE OSMANOĞLU HASTANESİ SAĞLIK HİZMETLERİ A.Ş. I have read and understood the
